Severe joint pain can be debilitating and limit one’s ability to function normally in daily life. Total joint replacements are commonly performed orthopedic procedures that can help restore mobility and improve quality of life in individuals with chronic joint pain. Although total joint replacements can be performed on many different joints throughout the body, the most frequently performed are hip and knee replacements. In fact, in 2015 it was estimated that nearly 7 million Americans were living with hip or knee replacements. There is an extensive recovery process associated with joint replacements that requires a lot of time and hard work. Physical therapy is a necessary component to optimize recovery during each phase of the process from beginning to end. Learn about the role physical therapy plays in total joint replacements by reading below!

Before Surgery

Did you know that you can have physical therapy before you’ve actually had your surgery? “Pre-hab” or pre-surgery rehabilitation helps to physically prepare you for surgery by engaging you in exercises to strengthen the joint that will be operated on and the surrounding muscle. Improving joint mobility and muscle flexibility will also be a primary focus before surgery. A physical therapist may also provide education on what to expect and ways you can modify your home for safety. Physical therapy before surgery has been shown to reduce surgical complications, time spent in the hospital after surgery, and overall recovery time. Being stronger with increased mobility and flexibility before surgery gives you the best chance to recover well after.

Immediately After Surgery

Oftentimes, people are able to return home the exact same day after the procedure, although this may vary based on pre-existing medical and post-operative conditions. Before being discharged, it is important that you are able to be as independent as possible. It is likely that just hours after your surgery, you will have your first physical therapy session! A physical therapist will get your post-op recovery started on the right foot by:

  • Providing education on post-operative care and any precautions or restrictions you may have
  • Helping you learn how to use an assistive device such as a walker or crutches and initiating walking
  • Teaching you how to safely perform transfers like getting into and out of the bed or a car
  • Instructing you in simple exercises to improve mobility and strength

Post-Op Rehabilitation

After you have returned home, the long-term recovery process quickly begins. Initially, the primary goals for physical therapy will be ensuring the surgical site heals properly, decreasing swelling, and managing pain while beginning to get the joint moving and muscles working. Once these goals have been met, your physical therapist will create a personalized plan of care to restore function and regain independence. This treatment may include: range of motion exercises, stretching and strengthening exercises, manual (hands-on) therapy, balance training, gait (walking) training, transfer training, and modalities like ice and electrical stimulation for pain management.
