Hip Weakness May Be Contributing to Your Back or Knee Pain

There are a number of reasons why someone may develop muscle weakness. Inactivity, poor muscle activation, and compensatory movement patterns are just a few of these reasons. But can weakness lead to the onset of, or contribute to pain? The evidence suggests that it can. Specifically, weakness of the hips can be part of the reason someone may be experiencing pain in other areas of the body, such as the knee or low back. 

Hip Weakness & Knee Pain

In order for the knee to function properly, it is important that the femur and tibia articulate with each other appropriately. Weakness of the hips can alter the mechanics of the knee which can lead to increased force placed through the joints when walking or exercising. Studies have shown that those who experience pain in the front of the knee will often demonstrate weakness in the gluteus maximus and gluteus medius. These muscles are responsible for extension and abduction of the lower extremity. Weakness in these muscles can lead to the caving in of the knee – known as knee valgus – which can place significant stress through the knee. By engaging in exercises that promote strengthening and increased activation of these hip muscles, the biomechanics at the knee can be improved and ultimately help to reduce pain felt in the knee. 

Hip Weakness & Back Pain 

Chronic low back pain is a condition that many people experience and can be due to a number of reasons. It is common for those with this pain to have weakness in the core musculature, but many will also show weakness of the hip abductors. Weakness of the abductors – a group of muscles responsible for elevating the lower extremity out to the side – can contribute to instability at the hips and make low back pain symptoms more prevalent. Studies have examined the effects of strengthening these muscles and their effects on low back pain. Those who underwent an abductor strengthening regimen for several weeks saw improvements in their reported pain and function. This suggests that, in addition to performing core strengthening, strengthening of the hips can have meaningful benefits for those who are looking to reduce their low back pain and improve their function.
