Thoracic Outlet Syndrome (TOS) is a condition caused by compression of the nerves, arteries, or veins in the passageway from the lower neck to the armpit, known as the thoracic outlet. This compression can lead to a variety of symptoms depending on the structures involved, including pain in the shoulder and neck, numbness or tingling in the fingers, and weakness in the hand or arm. TOS can cause anatomical abnormalities, trauma, repetitive movements, or poor posture. Diagnosis often involves a combination of physical exams, imaging, and nerve conduction studies, while treatment may range from physical therapy and lifestyle modifications to surgical intervention in more severe cases.

Physical therapy is a key component in the conservative management of thoracic outlet syndrome (TOS), particularly for the neurogenic type. It focuses on relieving compression in the thoracic outlet by improving posture, strengthening weak muscles, and increasing the flexibility of tight structures around the neck, shoulders, and chest. A physical therapist may guide the patient through specific exercises that stretch the scalene muscles, pectoralis minor, and upper trapezius while strengthening the shoulder girdle and postural stabilizers to support proper alignment. Manual therapy techniques and nerve gliding exercises can also be used to reduce nerve irritation and improve mobility. By addressing muscular imbalances and movement dysfunctions, physical therapy can significantly reduce symptoms such as pain, numbness, and tingling, and help prevent recurrence of TOS.

If you experience similar issues regarding your neck or shoulder, physical therapy can help properly evaluate and improve your symptoms.
